The Easterseals Connections behavioral health home isn’t a house, it’s a “home base.”  It is a service that assists Monmouth County residents with mental health issues to find connections to health care and other community services they want and need.

How it works!

We offer Care Coordination – A Care Coordinator can work with you to identify what you need and assist you in getting these services. Whether you’re looking for a job, a certain kind of doctor, or a place to live, we’ll help make the connection.

We help you get healthy – We offer a wide variety of health and wellness activities so you can take charge of your overall wellbeing. Our staff will help you make and keep appointments, stick to prescribed medication plans, find transportation, plan healthy meals, set exercise routines, and more.
