Pediatric Home Care Services

Pediatric Home Care services offers a range of care and support for children and their families, including, nursing visits, physical, occupational, speech therapy, and social work for children who need short-term skilled care.

Community Health Centers

Community Health Centers provide health care for all ages. This includes comprehensive primary care, dental care, prenatal care, nutritional and diabetic counseling, and HIV/AIDS services.

Prevention Resource Network

Prevention Resource Network (PRN) provides high-impact prevention services to assist HIV negative and HIV positive people at risk for getting or transmitting the disease.

County Councils for Young Children

The purpose of the County Council for Young Children is to provide opportunities for parents / families to come together with providers and other community stakeholders to identify and discuss the needs, concerns, aspirations, and successes pertaining to the health, education and well-being of their children.

Project R.E.A.L.

Project R.E.A.L. (Real Empowerment on All Levels) offers free social events that bring together the young LGBTQ+ community and builds peer support. Project R.E.A.L. also provides sexual health advocacy and serves as a point of contact between the client and community resources.

Early Intervention

Early Intervention is a family training program that helps families with children (newborn to age 3) meet their developmental milestones. Services include social work, parent-training, speech, occupational, and physical therapies.

Public Health Nursing

Public Health Registered Nursing services are available to those in need for health-related services, including, but not limited to, immunizations, safety, lead poisoning and parenting.

Health families

The Healthy Families Program supports pregnant women or parents/guardians through home visits where a specially trained Family Support Worker brings education and resources to parents and children.

School-Based Nurse Practioner Program

Nurse Pracitioners provide primary health care services at the school for children in Pre-K to high school. Services may include diagnosis and treatment of common childhood health issues, medication prescriptions, health screenings, and management of chronic diseases such as asthma and diabetes.

HIV Case Management Services

HIV Case Management provides home and community-based services to individuals living with HIV/AIDS. The program connects persons with medical and community resources for medical care.

School-Based Youth Services Program

School-Based Youth Services Program, also known as the SPOT, offers programs designed to encourage healthy youth development.

Mobile Outreach Clinic Program

Services include Registered Nurse (RN) assessments and education as well as care coordination with primary care providers for those who live in transient homes or are physically or mentally disabled.

Special Child Health Services

Special Child Health Services provides case management for children, from birth through 21 years, who have or are at risk for a lifelong physical, developmental, and/or behavioral condition that affetcts the child’s health and well-being.

Nurse-Family Partnership

Nurse-Family Partnership is a home-visitation program for first-time pregnant women. A Registered Nurse provides home-visits to help them meet these three goals.

Women, Infant & Children Supplemental Nutrition

Women, Infant & Children Supplemental Nutrition (WIC) participants receive nutrition education and counseling, vouchers for the purchase of nutritious food, and referrals for health care and social services.

Parents as Teachers

Through home visits, Parents as Teachers help parents understand child development and connects them to community resources such as insurance, transportation, food, baby items, and health care.
