What is the Housing Supports Program?

The program helps NJ FamilyCare (Medicaid) members of all ages who have both a housing need and a health condition.

Eligible members are connected to a housing supports provider that can provide services to help them:

  • Find a place to live
  • Move into housing
  • Stay in housing
  • Make home modifications or changes needed to support their health

These supports help members achieve housing stability so they can better manage their health and well-being.

Who is eligible to receive services?

A person is eligible if they meet all three of the criteria below:

  • Have NJ FamilyCare (Medicaid) health insurance
  • Have a housing need–like being homeless, risk of losing current housing, recently released from jail, unsafe or unstable living situation
  • Have a health condition–like diabetes, depression, pregnancy, substance use, physical disability

How do you obtain Housing Supports services?

  • Call the 1-800 number on the back of the insurance card and say you’re calling about making a referral to the new Housing Supports Program
  • Contact an agency providing Housing Supports Services in Monmouth and/or Ocean Counties

Types of Housing Supports Services:

  • Pre-Tenancy Services
    • Develop an individualized housing support plan to help member achieve their goals
    • Assist with the housing search and application process
    • Provide connections to resources aiding with housing costs and other expenses
  • Tenancy Sustaining Services
    • Develop an individualized housing support plan to help members achieve their goals
    • Assist with lease renewals and housing certifications
    • Connect members to financial resources and social services
    • Assist in addressing behaviors that may jeopardize housing
    • Assist in resolving disputes with landlords
  • Move-in Supports
    • Pay for the set-up of the new housing unit, to address needs identified in the person-centered care plan
    • Pay for the move and supporting the details of the move
    • Security Deposits
  • Residential Modifications and Remediations
    • Provide remediation services, including air filtration devices, asthma remediation
    • Modify home environment(ramps, handrails, grab bars)
    • Provide medically necessary heating and cooling 

What Services are NOT provided by the Housing Supports Program?

  • Payment for rent
  • Provision of or payment for housing vouchers
  • Funding to build housing
