MonmouthCares is the Care Management Organization for Monmouth County, New Jersey.

MonmouthCares is a nonprofit organization that is responsible for face-to-face care management and comprehensive service planning for youth and their families with complex needs.   We follow the Wraparound evidence-based model in delivering care management services in the community, which include but are not limited to, comprehensive assessment, Individual Service Plan design and implementation, advocacy, and family support, monitoring, and information management.  MonmouthCares is also responsible for local community resource development. Our System of Care

We are part of the NJ Children’s System of Care that helps youth and families with emotional, behavioral, substance use, and intellectual/developmental disability needs. The Department of Children and Families’ Children’s System of Care is a statewide program, serving children in all 21 counties in New Jersey. The System of Care is made up of local systems, which blend professional or formal services with informal supports and resources, including civic or faith-based organizations as well as community recreation programs. Child and Family Teams - For Family Support & Care

MonmouthCares sends surveys to enrolled families twice a year, and once a year send surveys to transitioned families, providers, and community stakeholders.

Our Health and Wellness Team

MonmouthCares has expanded our services, integrating a Health and Wellness team that works alongside our care managers to address chronic medical conditions along with their emotional, behavioral, intellectual/developmental, and substance use needs. This health and wellness team is comprised of a Nurse and Wellness Coach that joins each family’s team.

MonmouthCares' care management is funded through Medicaid and available to all youth and families that live in Monmouth County and meet the eligibility criteria of the New Jersey Children's System of Care.

Every family enrolled in MonmouthCares receives a copy of our Family Guide to Wraparound which details who we are and explains our Wraparound process.

Eligibility MonmouthCares serves under the age of 21. To be eligible for services, youth must manifest moderate to severe emotional or mental health challenges consistent with a DSM5/ICD 10 diagnosis. MonmouthCares also serves youth with developmental disabilities and/or substance use. Our care management services are delivered in the community. All community referrals to MonmouthCares must start with a call to PerformCare at 1-877-652-7624 to access services. 24-hours a day, 7 days a week. In case of emergency, call 911.
