This free program serves children who are homeless and includes youth at risk as follows: Homelessness, Substance Abuse, Dropout prevention, Teen pregnancy, Juvenile Justice populations, Poverty/family dysfunction, Chronic Disease

The services are FREE including the following:

Medical Evaluation, Dental Screening, Developmental assessment, Mental Health Assessment, Cognitive assessment, Trauma assessment, Immunization review, Care management plan development, and Communication with student providers.

Immunizations and TB testing may be provided for school-age children

Transportation may be provided.
