Healthy Connections is a program that assists individuals in the community in overcoming the medical, behavioral, and social barriers that are preventing them from being healthy. Support from our Community Health Workers can look like getting the individual connected to the appropriate resource or provider to help them overcome their health-related concerns. Community Health Workers can provide support telephonically, over e-mail, meet with individuals in the community, and if necessary, perform home visits.

Support from a CHW could look like finding a new primary care provider or specialist, connecting with a therapist, or to help address social needs, support in navigating community resources that can help with utility and rental assistance, connecting with food pantries, searching for employment or going back to school. Community Health Workers are neighbors in the community who take pride in supporting individuals overcome barriers that are preventing them from living their healthiest life.
