Special Child Health Services provides case management for children, from birth through 21 years, who have or are at risk for a lifelong physical, developmental, and/or behavioral condition that affects the child’s health and well-being. A nurse/social worker case manager assists the family with finding programs and services to help their child.

Eligibility	Any Monmouth County resident with concerns about their child can be referred.
