Immunization Services for Children (18 and Under)
Childhood immunizations are provided free to children from birth to 18 years old who have no insurance or are uninsured, living in Monmouth County Health Department Member Towns. With parental permission, the child/children will be entered into a statewide immunization registry, which encourages timely and age appropriate immunizations.

The clinic is run by APPOINTMENT only. It is open every Monday, except holidays from 9-11:30 a.m. and 1:00 p.m.-3:30 p.m. The appointment can be made by calling either 732-294-5458 or 732-431-7456, x 8516.

Patient must be accompanied by a legal parent and/or guardian; identification must be shown.

If the School Nurse completes his or her assessment of patient records (ex. out of country or state), please attach a copy of those documents for the patient to present at clinic. If no documentation is presented at school, please make a note on your form and/or letter that accompanies the patient at clinic.

Should clinic be closed, we make every effort to update our phone message, but please understand emergencies do happen.
