CPC Integrated Health is a nonprofit organization with locations throughout Monmouth County, NJ. CPC accepts Medicaid, Medicare and most insurances and has grant funding for individuals and families who are uninsured.

Outpatient Treatment for Mental Health, Substance Use and Co-occurring Disorders for Children and Adults

• Psychiatrists and nurse practitioners provide psychiatric evaluation and medication management for children and adults with mental health and substance use disorders
• Individual, group and family counseling with licensed clinicians
• Care management services
• Intensive outpatient (IOP) services for individuals with substance use disorders
• Medication for Opioid Use Disorders (MOUD) and Alcohol Use Disorders (MAUD)
• All of these outpatient services available at all five CPC locations

Adult Partial Care Program

Full or half day programming in psychiatric rehabilitation, individual and group counseling, and care management services. Available at CPC Aberdeen Counseling Center. Transportation available

Integrated Case Management Services

In this program, we assist adults with mental illness and/or co- occurring disorders re-integrate into the community, providing linkages and resources. Special programming is offered to individuals with involvement in criminal justice system.

Supported Employment

In this program we assist adults with their goal to return and remain in the workforce, providing a continuum of vocational services.

Supportive Housing

In-community care management for adults with the goal of increasing independence, integrating into the community, and improving quality of life.

PACT

PACT is Community treatment for individuals with serious mental illness and substance use disorders through a team-based approach, including a psychiatrist, nurses, clinical social workers, co-occurring specialist, vocational specialists, mental health advocates and peer specialists.

Children’s Mobile Response and Stabilization Services

This program offers 24-7 crisis deescalation, safety planning and case management supports to children and families in Monmouth County within 1 hour of their call. Families should call the System Administrator, PerformCare at (877) 652-7624 for more information and linkage to services.

CPC High Point School

Approved, private special education for children ages 5 to 21 who learn best outside of the traditional school setting. Clinical and supportive resources reinforce academics to provide each child the opportunity for personal growth. Class sizes limited to 12 with a student to staff ratio of 3:1.
