Central Jersey Family Health Consortium (CJFHC)

CJFHC is formerly known as the Central New Jersey Maternal & Child Health Consortium and The Regional Perinatal Consortium of Monmouth & Ocean Counties (they are now officially joined as one). The CJFHC will continue their work toward improving the health & well being of women, children, and families in Central New Jersey through the collaborative efforts of member hospitals, health care providers, community organizations, governmental agencies, and consumers. Their primary function is to assist with the orderly provision and coordination of all hospital and community-based maternal and child health services within their regions. This includes patient care from the prenatal phase through delivery and pediatric primary care. These services include: • coordination of high risk perinatal/pediatric transport services • data analysis • development of comprehensive perinatal/pediatric regional plans • high risk infant and pediatric follow-up outreach • planning for improved access to service • prevention activities • consumer and regional professional education • total quality improvement
